In a summary statement of deficiencies dated 06/09/2015, a complaint investigation against the facility was opened for its failure to "notify the physician [in a timely manner] of a change in the resident’s condition.” The deficient practice by the nursing staff at Generations at Rock Island were first noted in the facility’s 05/24/2015 7:03 PM Nurses Note the documents that a Certified Nursing Assistant (CNA) providing the resident care “alerted the nurse to the resident’s ‘red vaginally area’ upon examination [by a nurse who noted the] resident’s labia (right side) is red and swollen. Origin of swelling and redness is unknown, will pass information to oncoming shift to monitor and have [the facility’s Nurse Practitioner] examine the area in the [morning].”
Follow-up Nurse’s Notes at 12:46 AM on May 20 15,015 indicated the resident’s “vaginally area remains red and swollen and very tender to touch. Tylenol given for discomfort.” A member of the nursing staff asked a resident’s nurse at 3:05 PM on 05/29/2015 after supper to “come look at [the resident who was] acting withdrawn and fearful.” It was noted that the resident’s “the genitals were red and swollen [… with] bruising on the [resident’s] inner thighs.” The nurse reported that the resident “had a bruise to the right eye, around the cheekbone.” A decision was made to report it to the proper authorities.”
The Nurse Practitioner examined the resident at 3:50 PM on 05/29/2015 after getting a call 6:30 AM on Monday, 05/25/2015 after being told “there was bruising to [the resident’s] upper extremities, inner thighs, right eye and swelling and redness to [the resident’s] vaginally area.” The Nurse Practitioner said that “if I knew the way [the resident] was acting, how [the resident] was withdrawn and fearful along with all the bruising and scratches, I would have sent [the resident] to the emergency room that night to be assessed for sexual assault.”
- Failure to Develop, Implement and Enforce Policies That Prevent Abuse and Mistreatment of Residents
In a summary statement of deficiencies dated 06/09/2015, the state investigator noted the facility’s failure “to follow’s policy on abuse prevention by not immediately reporting an incident of potential sexual assault to the facility Administrator, physician, law enforcement, and responsible party. The facility failed “to follow their policy and investigate timely an incident of potential sexual assault involving [one resident at the facility].”
