In a summary statement of deficiencies dated 01/08/2015, a complaint investigation against the facility was opened for its failure to "ensure a prescribed treatment to heal a pressure ulcer was applied as ordered for [one resident at the facility].”
The deficient practice was first noted in the state investigator’s findings after a review of a resident’s 10/29/2015 MDS (Minimum Data Set) revealing that the cognitive, alert resident “requires the total assist of two staff for transfers." The resident’s 10/06/2015 Clinic Dressing and Continuum of Care Form completed by the facility’s Wound Care Specialist “indicates treatment for Stage IV, 3.6 centimeters by 1.6 centimeters by 2.8 centimeters deep pressure ulcer of the left ischium, prior to admission to the facility." The nursing staff was directed to change the resident’s dressing “three times a week and as needed for soiling/dislodged.”
An observation was made up to Certified Nursing Assistant in the early afternoon of 01/04/2015 transferring the resident “from a wheelchair to the bed with a mechanical lift. There was a red stain that appeared to be serosanguinous, approximate four inches in diameter on the sling of the mechanical lift approximate to the pressure ulcer on [the resident’s] left buttock." The investigator noted that when the resident was rolled to their side by the Certified Nursing Assistants, there “was no dressing on the pressure ulcer.”
The investigator interviewed the facility Director of Nursing on the afternoon of 01/07/2015 who stated “there is no reason for [the resident] to be without a dressing to a Stage IV wound on her left buttocks [...and] stated the CNA staff caring for [the resident] should have informed the nurse immediately, so the dressing could have been replaced."
- Failure to Notify a Resident’s Physician of a Change in the Patient’s Condition Including a Decline in Their Health or Injury
In a summary statement of deficiencies dated 10/21/2014, the state investigator noted the facility’s failure to “notify the physician of a continued decline in the condition of [one resident].” The failure involved a patient whose health was declining over the weekend. Even though the nursing staff at notified the physician early Saturday morning, the doctor had not been updated throughout the weekend that the resident’s family “had taken her to the local emergency room on Sunday."
