In a summary statement of deficiencies dated 06/17/2016, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "ensure resident incidences were investigated to prevent recurrence.” The state investigator also noted numerous other failures that included:
- “The facility repeatedly failed to investigate and record events where lift slings failed during transfers.
- The facility failed to ensure policies and procedures for falls, accident reporting, and manufacturer’s guidelines were followed.
- The facility failed to ensure the structural integrity of the lift slings before using a sling during transfers for mechanical lifts.”
It was noted that “these failures resulted in a sling breaking while [a resident] was being transferred on 06/06/2016.” The deficient practice by the nursing staff at Franklin Grove Living and Rehabilitation Center resulted in the resident falling and sustaining “fractures to both her lower extremities and was hospitalized on 06/06/2016.” The resident’s “fall contributed to her death on 06/10/2016.
As a part of the investigation process, it was revealed that “the neglect existed between 05/11/2016 and 06/17/2016. An Immediate Jeopardy began on 05/11/2016 at 3:00 PM. The Administrator was notified of the Immediate Jeopardy on 06/15/2016 at 9:15 AM. While the immediacy was removed on 06/17/2016 at 10:00 AM, the facility remains out of compliance at Severity Level 2.” The investigator noted that “additional time is needed to monitor and evaluate the effectiveness of the revised policies and procedures to ensure their implementations.”
- Failure to Maintain the Dignity and Respect of Every Resident’s Individuality
In a summary statement of deficiencies dated 03/15/2016, a complaint investigation was opened against the facility for its failure to "maintain a resident’s dignity by ensuring a feeding tube was covered while seated in a common area.”
The state investigator noted upon observation of a resident on 03/10/2016 while the resident “was in the day room with her feeding tube hanging outside her clothing in full view of passersby between 7:45 AM and 9:30 AM.” It was also noted that during that timeframe, the resident “had a pastor visit, [the Activity Director] sat next to [the resident] and a man playing a guitar and singing sat next to her as well.”
The investigator conducted an interview with the facility’s Director of Nursing on the morning of 03/10/2016 who said “feeding tubes should be covered when a resident is in a common area to respect their dignity.” It was also noted that the actions by the nursing staff failed to follow the facility’s undated Dignity Policy that reads in part:
“Privacy of the resident’s body must be maintained while in common areas of the facility and staff will ensure dignity is preserved.”
