In a summary statement of deficiencies dated 12/03/2015, a complaint investigation against the facility was opened for its failure to "provide to residents with comprehensive psychosocial rehabilitative services needed to address her socially inappropriate behaviors between peers, staff, and provide one resident with training skills to cope with possible independent living, and discharge from the facility.” The state investigator noted that the deficient practice by the nursing staff at Frankfort Terrace Nursing Center applied to two residents at the facility “who have a need for mental health rehabilitative services.”
As a part of the investigation, the state surveyor reviewed the resident’s November 2015 and December 2015 Physician Orders Sheet (POS) that revealed the resident “is being treated with… antipsychotics.” In addition, the resident’s 09/17/2015 Level of Functioning Scale Assessment reveals of the resident “needed supervision with self-maintenance [...and] had problems with maintaining friendships, socializing and interacting with others [...and] this assessment identified [the resident] had poor conflict avoidance skills and behaviors of threatening, screaming, cursing at others.”
A review of the resident’s 09/12/2015 psychosocial Care Plan identified that the resident “had behaviors of inappropriate sexual behaviors, manipulation, being verbally and physically abusive [...and the resident’s] Care Plan did not identify specific way staff was to monitor for these targeted behaviors and intervene before the behaviors are exhibited toward other residents.” The investigator also noted that the resident’s “care plan did not identify the social coping skills the nursing staff should encourage [the resident] to use [...and] did not identify what symptoms of [the residents] behaviors may signal decompensation in his condition before conflicts with peers.”
The investigator noted five documented incidences of inappropriate behavior with others including making “sexually inappropriate comments in the dining room” on 09/26/2015, being “physically and verbally aggressive” on 10/11/2015, “yelling at a peer as she sat in a chair” on 10/20/2015, “yelling at staff” on 11/04/2015 and “calling a co-peer a fat b----, because she was on the phone and he wanted it to call his mother [so he] snatched it out of her hand” on 11/18/2015.
Documentation recorded on 11/13 2015 show that the resident “expressed a desire for transfer to another facility and go to [another] community.” However, the two assessments say that the resident “is ready, but gave no details of the rehabilitative services given to make the dramatic change in his condition.” The investigator also noted that “the interventions are not specific to an individualized [in the resident’s Care Plan] in identifying what staff is doing to prepare [the resident] and help them cope with a possible discharge or transfer to another facility.”
