The Appalling Discoveries Regarding Patient Care at Franciscan Village
The surveys conducted by the Illinois Department of Health and other private health inspection firms discovered different episodes of poor patient care, medication errors and abuse at the health center which could be interpreted as nursing home negligence. - Failure to keep residents free of neglect: “Based on clinical record review, review of hospital records, review of EMT (emergency medical transportation) records, review of facility’s policy r/t significant changes, interviews and observation the facility failed to ensure that 1 resident was free from neglect as evidenced by the facility’s failure to: 1. Monitor and comprehensively assess a resident (R2) who had increasing lethargy, sluggishness and decreased responsiveness over 48 hours in the facility. 2. Notify the physician of this medical change at the onset of these symptoms. 3. Provide evidence that a resident received pain medication as ordered by the physician including one topical pain patch being removed before another one is applied. 4. Ensure an accurate and complete assessment, monitor, document, and evaluate the effectiveness of pain medication given for chronic pain including adverse effects. These failures resulted in R2 becoming increasingly sleepy/lethargic over a 24-48 hour period while at the facility. EMT (emergency medical transportation) was called to transfer R2 to a local hospital, however while preparing for transfer EMT staff found four pain patches ([MEDICATION NAME]) on R2 and informed the nurse. [MEDICATION NAME] (narcotic antagonist) intravenous push was required to stabilize R2 enroute to the hospital. R2 was admitted to the ICU (intensive care unit) of this local hospital with [DIAGNOSES REDACTED].”
- Failure to identify residents correctly prior to administering medication: “This failure resulted in a drop in R1’s blood pressure resulting in an emergency hospital admission. The facility incident report of 7/31/09 completed by (E7/Assistant Director of Nursing) in part denotes, “R1 came up to nurse (E3/charge nurse) for AM meds, E3 asked are you R2? The Resident (R1) who has Dementia, stated, Yes I am. (R1) was in fact (R2). E3 gave R1 the medication for R2….R1 was admitted to the hospital with [DIAGNOSES REDACTED]. E8 (Administrator) interviewed on 8/19/09 stated,” all residents have pictures, we use for ID, nurses are supposed to check the pictures before giving medications. We have taken new pictures and inserviced all staff since the incident. We have taken new pictures and inserviced all staff since the incident.”
- Failure to remove medication patches as indicated by physician order: “According to the ambulance EMS report, at 2:57pm R1’s blood pressure was recorded at systolic 80/p, and the comments section of the EMS report indicates that that (2) [MEDICATION NAME] were found on R1’s body. According to the hospital record initial assessment tool dated 3:48pm, R1 was brought to the emergency room related to altered mental status. However the assessment tool indicates that there were 2 [MEDICATION NAME] found on R1’s body. The assessment tool indicated the patches were removed related to low blood pressure. According to the emergency room initial assessment tool, R1’s first blood pressure was recorded at 76/54, heart rate 128, respirations rate 26, and temperature of 102.9 degrees. R1’s mental status was assessed to be lethargic, and unresponsive, and skin assessed to be hot, pale and dry.”
