    In data gathered from a summary statement of deficiencies dated 11/30/2012, a complaint was filed against the facility indicating that the resident’s doctor, legal representative or family member was not immediately informed of a serious decline in the resident’s health status. This includes a deterioration of their health, a clinical complication or life-threatening condition.

    On 9/20/12, the Nurses Notes indicate the resident had developed a facility-acquired pressure ulcer to the “lateral aspect of the right heel” and to the “lateral aspect of the left heel” that had 100% necrosis by the following day. In addition, “There was no documentation to show the facility notified [the resident’s] family … of these pressure sores.”

    On 11/16/12, “the facility administrator stated that [the resident’s] family was difficult to deal with and the facility staff spoke with the family almost daily, but [the administrator] could not show documentation if the staff notified the family of these pressure sores.”
