The facility’s 04/09/2015 6:30 PM Incident Report revealed that the resident “was found on the floor in the second-floor main dining room. The report shows that [the resident] had complained of pain on her right arm during the post-fall assessment [...and] was sent to the hospital for further evaluation.” The incident report revealed that “the resident was not admitted to the hospital and had returned back to the facility [noting that] the root cause analysis for [the resident’s] fall was that the fall might have been related to/caused by/probably caused by: [the resident was] confused, noncompliant with care and noted to try to get up without calling for assistance.”
In an interview with the Director of Nursing at 4:00 PM on 02/08/2016, it was noted that the resident’s 03/02/2015 Care Plan with revision dates of 03/10/2015 and 05/26/2015 “show there was no intervention to prevent possible skin injury when [the resident] was unable to maintain good alignment on her lower extremities.”
- Failure to Ensure That a Resident Remained Accident-Free in Their Environment and Was Provided Adequate Supervision to Avoid an Accident from Occurring
In a summary statement of deficiencies dated 06/10/2015, a complaint investigation was opened against the facility for its failure to "ensure that personal bed/chair alarm is activated to ensure resident safety.” The deficient practice by the nursing staff at Elm Brook Healthcare and Rehabilitation Center affected 17 residents at the facility who were “reviewed for falls.”
The state investigator noted that during a routine observation between 4:20 AM and 5:30 AM on 06/09/2015, conducted on 34 of the facility residents, there were 15 residents observed “on their various beds and in their rooms with their bed alarms and activated. This occurred in various resident rooms on different floors.”
The state investigator interviewed a facility’s Certified Nursing Assistant at 4:50 AM that same day, who stated that “she was not sure why bed alarms were off for residents but stated bed alarms were supposed to be checked every shift by nursing staff.”
