In a summary statement of deficiencies dated 08/31/2011, notation is made in an annual certification survey indicating that the facility “failed to follow its sanitation policy to wash, and sanitize utensils by approved methods.” This failure also included a failure “to wear gloves appropriately and failed to wash hands after moving kitchen equipment into the serving area.”
This is in response to a surveyor observing four different occasions where a lunch service worker was seen “washing her hands with plain water and washing the ladle she was using at the same time.” The worker also “use a towel to dry the ladle. At no time was [the worker] observed soaking the ladle in sanitizer, prior to reusing the ladle and failed to wash her hands prior to applying gloves.”
In a separate occasion, in a summary statement of deficiencies dated 07/16/2009 during an annual certification survey, it was noted that the facility “failed to ensure 1) to store food properly and labeled foods 2) serve food at the proper holding temperature and 3) to sanitize the dishes with heat sanitation appropriately.”
- Failure to Provide Residents and Environment Free of Physical Restraints:
In a state summary dated 8/7/03 during an annual certification, it was noted that “the facility failed to document and show the specific medical symptoms for the application of restraints, how the restraints would treat the medical symptom, what alternative methods have been tried, and if reduction plans for the use of restraint [for a resident].” This notation is in response to use of restraint on a resident diagnosed with dementia with agitation, osteoporosis and kyphosis who was admitted and treated the facility without proper assessment of the need to use restraints.
