The investigator noted that the “management technique that results in, or has the potential to result in, physical or psychological harm.” The failure was first documented at 10:30 AM on 08/12/2015 when a resident stated that the Registered Nurse “is mean and yells at me. She makes a full out of you in front of other residents. It makes me feel terrible, cheap and less of a human. They call [the Registered Nurse] the B***h. When I get done eating, I am expected to stay in the dining room and wait for [the Registered Nurse] to administer medications, like this is her house. If I want to lie down or use the bathroom, [the Registered Nurse] tells me I will just have to wait. By the time I medications are given, there is nobody available to help me and I’m stuck out there like an idiot.”
The state investigator interviewed a Certified Nursing Assistant working at the facility at 10:50 AM on 08/11/2015 who stated that the Registered Nurse “has been unprofessional in exchanges with residents and abusive in her appropriate tone of voice toward residents. [The Registered Nurse’s] behavior has been escalating and she continues to come to work.”
The Certified Nursing Assistant also claimed that on a day when the facility “was testing sprinklers and alarms” causing a buzzing, humming sound most of the day, a resident “repeatedly asked for help with her hearing aids and she thought they were causing the noise.” The resident asked the registered nurse to check your hearing aids and was told: “in a louder sharp voice to go sit down and again repeated she had had it with [the resident].” When the resident requested help with her hearing aids at the nurses’ station, the Registered Nurse “removed the hearing aids from [the resident’s] ears.”
- Failure to Report and Investigate Any Act or Allegation of Abuse and Mistreatment of Residents
In a summary statement of deficiencies dated 08/14/2015, the state investigator noted the facility’s failure to “ensure staff immediately report allegations of abuse and the facility failed to investigate allegations of abuse.” The state investigator noted that the facility failed to perform any abuse investigation concerning the Registered Nurse who had been “unprofessional in exchanges with residents and abusive by her inappropriate tone of voice toward residents” is documented by the Certified Nursing Assistant at the facility at 10:50 AM on 08/11/2015.
