The survey indicates that the resident experienced “labored breathing for five hours (from 8 AM through 1 PM)” and waited for an “ambulance for an hour (from the time the order was given at 12:00 PM through the ambulance arrival time at 1:00 PM).”
- Failure to Fully Inform the Patient of His or Her Total Health Status in an Understandable Language:
Information obtained in a summary statement of deficiencies dated 08/06/2014, a patient filed the complaint indicating that the facility failed to fully inform them of their current medical condition and total health status in a language he or she could understand. On 8/01/13, the patient was prescribed a psychoactive medication that required a prior informed consent. A review of the resident’s “electronic medical record did not showed that a consent was ever obtained for this psychoactive medication. Consent wasn’t obtained by the facility from [the patient’s] Power Of Attorney for healthcare (POA) for this drug until 8/5/14.”
- Failure to Perform Nursing Interventions Which Led to a Resident’s Falling:
Information obtained in a summary statement of deficiencies dated 10/11/2013, the nursing staff at the facility neglected to initiate a developed care plan for residents suffering from cerebrovascular disease and dementia, which led to the patient falling on 8/23/13 and again on 9/23/13. Even though it was determined that the resident was a fall risk in July 2013, the “plan of care failed to showed any revision in the use of safety devices. The use of alarms were found to be ineffective because [the resident] removed the alarms without making a sound. The resident suffered serious injuries to her head, left forearm and right knee from falling.”
