- Failure to Provide a Residential Environment Free of Accident Hazards
In a summary statement of deficiencies dated 07/23/2015, and incident investigation was opened concerning the facility’s failure “to ensure hazardous chemicals are stored securely away from cognitively impaired, wandering residents.” This is in response to a 7/22/15 notation that a “deficit practice has the potential to affect six out of 15 residents who were ambulatory and assessed as cognitively impaired.” The incident involved a bottle of “Miracle Liquid Feed Miracle Grow in a green model that resembles a soda bottle. The chemical was sitting in a box on the patio and was accessible to the residents.”
In a second summary statement of deficiencies dated 09/11/2014 incident from a certification annual survey notes that “the facility failed to provide effective interventions to prevent falls for [a resident].” This is in response to a resident diagnosed with general weakness, dysfunction, expressive aphasia and cerebral vascular accident (CVA). The resident “has poor balance, is incapable of safety judgment, attempts to climb out of bed and get up from wheel chair unassisted, is unable to express self clearly, and is unable to learn.”
The notation refers to a review of facility accident / incident log documents indicating the resident “has had 17 falls between 5/28/14 and 9/6/14.” Despite numerous interventions being in place, the resident “continues to have multiple falls.”
- Failure to Properly Investigate an Allegation of Abuse
In a summary statement of deficiencies dated 10/25/2015, a notation is made that the facility “failed to report an allegation of abuse immediately to the Administrator, allowed a Certified Nurse Aid (CNA) to have direct contact with residents after potential abuse incident and failed to do a thorough investigation of allegation of abuse [of a resident].”
