The resident “has poor balance, is incapable of safety judgment, attempts to climb out of bed and get up from wheel chair unassisted, is unable to express self clearly, and is unable to learn.”
The notation refers to a review of facility accident / incident log documents indicating the resident “has had 17 falls between 5/28/14 and 9/6/14.” Despite numerous interventions being in place, the resident “continues to have multiple falls.”
- Failure to Properly Investigate an Allegation of Abuse
In a summary statement of deficiencies dated 10/25/2015, a notation is made that the facility “failed to report an allegation of abuse immediately to the Administrator, allowed a Certified Nurse Aid (CNA) to have direct contact with residents after potential abuse incident and failed to do a thorough investigation of allegation of abuse [of a resident].”
