The state investigator noted the resident’s 12:35 PM 08/12/2015 Meal Ticket that was on the resident’s “tray in his room that shows aspiration precautions.” It was noted at 8:56 AM, “the door was closed and [the resident] was in his room eating breakfast. There was no staff in the room with him.” A Registered Nurse at that time stated that the resident “was a slow eater and that he has problems swallowing and chewing.” On 08/12/2015 at 12:35 PM, the door was [again] closed and [the resident] was in his room eating lunch.”
During the observation, it was noted that the “head of the bed was at 30 degrees.” The resident “was having a difficult time eating his lunch […and] could not reach his plate […and] asked for help. [The] surveyor went to the doorway and notified [the Registered Nurse/Restorative Nurse who] put the head of his bed up right away, saying your head should be up higher.” At that time, the Certified Nursing Assistant (CNA) “came in to help” the Registered Nurse. The resident “then was sitting up 90 degrees in the bed and could reach his food better.” However, the surveyor noted that both the Registered Nurse and the Certified Nursing Assistant “left the room and no staff were in the room while [the resident] continue to eat his lunch.” The resident “started coughing twice.”
At 9:05 AM on 08/13/2015, in an interview with the Director of Nursing facility, it was revealed that “if anybody is eating in their room, then staff/CNA should be in the room with them the entire time they are eating.”
