  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Avoid Injury In a summary statement of deficiencies dated 09/22/2016, a complaint investigation against the facility was opened for its failure to “provide a safe environment and follow a resident’s Plan of Care for risk of falls and/or injuries.” The deficient practice by the nursing staff affected one blind resident who “was noted to have her roommate’s fall mat folded up next to the bathroom doorway in [the resident’s] pathway. At 1:50 PM and at 3:20 PM, the same floor mat was in the same place and position.”During an interview with the blind resident on 09/21/2016, it was stated that “it was not the first time something had been placed in her way or in her walking path and no one made her aware.” The resident stated that she “was totally blind and unless someone made her aware, if something was there she had no way of knowing […and] stated more than once she has almost fallen because of items in her walkway […and] stated she has spoken to several Certified Nursing Assistants and nurses about her concerns.”

    During the interview, the resident stated “she had also talked to the Administrator last week and thought things would have been taken care of and changed but they did not, and the problems continued.” In the resident’s room at 8:50 AM, “it was noted that the third drawer of her chest of drawers was over half way out.” The resident stated that “she wasn’t aware of this.” The dresser drawer was noted to remain in this position at 11:15 AM.”

    The investigator reviewed the resident’s 10/21/2015 Care Plan developed nearly a year earlier that revealed documentation of an intervention is to “educate staff that pathway from bed, closet and pathway [remains] clear.” It also states that “the goal [of the plan is to ensure that the resident] will feel safe in her living environment and identified intervention is: Identify medical, psychosocial and environmental needs…”

    The state investigator interviewed the facility Director of Nursing that morning at 11:30 AM who stated “the fall mat should not be kept next to the door in [the resident’s] room because it is a safety issue and would increase her risk for falls.” The Director also stated “the staff is to place the fall mats under the bed or at the head of the bed if the resident is out of bed and not using them for safety issues.”

    As a part of the investigation, it was noted that “the facility must ensure that the resident environment remains as free from accident hazards as is possible, and each resident receives adequate supervision and assistive devices to prevent accidents.”
