Rosewood Care Center of RockfordTypically, weakness, poor health, sedation, paralysis or illness in a nursing facility can lead to the development of pressure sores. However, through proper monitoring and skin care management by the medical team, most bedsores (decubitus ulcers; pressure ulcers; pressure sores) can be prevented. Advanced stage bedsores are usually an indicator of poor quality health care management at the nursing facility by the staff in charge to provide appropriate care.

The Rockford nursing home neglect attorneys at Rosenfeld Injury Lawyers LLC have found numerous complaints and opened investigations involving Rosewood Care Center of Rockford that include:

  • Failure to Provide an Environment for Residents Free of Pressure Sores:

In a summary statement of deficiencies dated 04/10/2013, a complaint investigation was opened noting that the facility “failed to apply skin preparation and offload pressure of [a resident’s] heels.” This is in response to the facility’s nurse’s notes dated 04/05/13 indicating that the resident was “seen by a wound specialist physician on 4/13/12; Unstageable deep tissue injury on left medial heal, 2.0 cm x 2.2 cm, no exudate noted. Encourage the use of offloading boots.”

Although there were specific physician’s orders to use offloading boots, upon observation on 4/5/13, no offloading boots had been used. At the conclusion of the investigation, it was noted that the facility failed to implement monitor preventative protocols on a consistent basis.

  • A Failure to Provide Residents an Environment Free of Physical Restraints:

In a summary statement of deficiencies dated 02/05/2015, during an annual licensure and certification survey, notations were made of the facility’s failure to “reassess the use of restraint for a resident who had a condition change. The facility failed to have release times when using a seatbelt restraint.” This investigation was opened in response to a resident being reviewed for restraints that showed on 02/03/15 during three separate observations at 7:45 AM, 9:10 AM, 11:00 AM showing that the resident was “sitting in a reclining chair with a wedge cushion and a seatbelt secured around her waist.”

On the same day, at 12:40 PM, the resident “was in her room sitting in a reclining chair with a wedge cushion in a seatbelt secured around her waist, while a staff member was feeding her.” Again at 2:00 PM, the resident “remained in a reclining chair with a wedge cushion and seatbelt on. Her care plan implemented to last through April 2015 indicated that there were “no release times for the seatbelt are identified.”

Client Reviews

Jonathan Rosenfeld was professionally objective, timely, and knowledgeable. Also, his advice was extremely effective regarding my case. In addition, Jonathan was understanding and patient pertaining to any of my questions or concerns. I was very happy with the end result and I highly recommend Jonathan Rosenfeld.

- Michonne Proulx

Extremely impressed with this law firm. They took control of a bad motorcycle crash that left my uncle seriously injured. Without any guarantee of a financial recovery, they went out and hired accident investigators and engineers to help prove how the accident happened. I am grateful that they worked on a contingency fee basis as there was no way we could have paid for these services on our own.

- Ethan Armstrong

Jonathan helped my family heal and get compensation after our child was suffered a life threatening injury at daycare. He was sympathetic and in constant contact with us letting us know all he knew every step of the way. We were so blessed to find Jonathan!

- Giulia

This lawyer really helped me get compensation for my motorcycle accident case. I know there is no way that I could have gotten anywhere near the amount that Mr. Rosenfeld was able to get to settle my case. Thank you.

- Daniel Kaim

Jonathan did a great job helping my family navigate through a lengthy lawsuit involving my grandmother's death in a nursing home. Through every step of the case, Jonathan kept my family informed of the progression of the case. Although our case eventually settled at a mediation, I really was impressed at how well prepared Jonathan was to take the case to trial.
