Encourage the use of offloading boots.”
Although there were specific physician’s orders to use offloading boots, upon observation on 4/5/13, no offloading boots had been used. At the conclusion of the investigation, it was noted that the facility failed to implement monitor preventative protocols on a consistent basis.
- A Failure to Provide Residents an Environment Free of Physical Restraints:
In a summary statement of deficiencies dated 02/05/2015, during an annual licensure and certification survey, notations were made of the facility’s failure to “reassess the use of restraint for a resident who had a condition change. The facility failed to have release times when using a seatbelt restraint.” This investigation was opened in response to a resident being reviewed for restraints that showed on 02/03/15 during three separate observations at 7:45 AM, 9:10 AM, 11:00 AM showing that the resident was “sitting in a reclining chair with a wedge cushion and a seatbelt secured around her waist.”
On the same day, at 12:40 PM, the resident “was in her room sitting in a reclining chair with a wedge cushion in a seatbelt secured around her waist, while a staff member was feeding her.” Again at 2:00 PM, the resident “remained in a reclining chair with a wedge cushion and seatbelt on. Her care plan implemented to last through April 2015 indicated that there were “no release times for the seatbelt are identified.”
