In a summary statement of deficiencies dated 12/03/2015, a complaint investigation was opened against Renaissance at Midway for its failure “to follow policy regarding hand hygiene for [a resident].” This is in response to a 12/1/15 incident where a certified nurse’s aide “transported [a resident] by wheelchair to his room, [the nurse’s aide] stated in part, the resident has just returned from a day program I will check him and put him into bed. [The certified nurse’s aide] applied gloves to remove [the resident’s] coat and clothing and transferred [the resident] to bed by mechanical lift with assistance from [another certified nurse’s aide]. While the certified nurse’s aide removed the resident’s incontinence brief and provided perineum care, it was done without changing gloves, which is in direct violation of proper hand washing/cleaning protocols the facility involving incontinence care.
In a separate summary statement of deficiencies dated 03/13/2015, a complaint investigation is opened against the facility for its failure “to provide indwelling urinary catheter care and incontinence care in a manner to prevent cross-contamination for [a resident].” This is in response to care provided by a registered nurse and certified nurse’s aide who “completed indwelling catheter care and incontinence care for [a resident]. [The certified nurse’s aide] cleaned the feces in an upward motion toward the urinary meatus spreading feces around the indwelling catheter tubing. [The resident] began yelling ‘that hurts, stop that’.”

In a summary statement of deficiencies dated 03/13/2015, a complaint investigation was opened against the facility for its failure “to implement pressure relieving methods to facilitate healing for [a resident].” While the resident’s care plan for alteration and skin integrity documents were complete, the facility failed to adequately follow or implement the physician’s orders.
