  • Failure to Provide Acceptable Standards of Care That Prevent the Spread of Infection In a summary statement of deficiencies dated 08/18/2016, a notation was made by a state investigator while performing an annual licensure and certification survey involving the nursing home’s failure to “ensure that handwashing was performed during wound care for residents.” The state investigator also noted the facility’s failure “to ensure the isolation cart for infected residents contained personal protective equipment (PPE).” The deficient practices by the nursing staff affected four residents at the facility “reviewed for wound care and infection control practices.”One failure was first noted after reviewing a resident’s August 2016 Physician Order Sheet (POS) revealing that in August 2016, a telephone order from the physician was documented “to clean the sacrum with normal saline and apply DuoDERM every three days.

    An observation was made at 1:40 PM on 08/17/2016 when a Licensed Practical Nurse (LPN) “was observed performing wound care treatment on [the resident].” The LPN was assisted by a Certified Nursing Assistant who helped position the resident “on her left side while being held by [the CNA].” After removing the wound dressing, the resident “had a bowel movement.”

    The Licensed Practical Nurse “washed her hands, donned on gloves, and sprayed and cleansed the sacral wound with normal saline [solution]. The LPN then changed her gloves without handwashing.” The Licensed Practical Nurse then “dabbed the wound with dry gauze, and change gloves without handwashing. The right-hand glove ripped at the wrist and a strip was left hanging while [the LPN] was putting on the glove.” At that time, the LPN “did not change the right-hand glove and the strip was touching the buttock” while the Licensed Practical Nurse “finished applying the DuoDERM.

    An additional observation was made at 1:45 PM on 08/17/2016 when the Wound Treatment Nurse “provided wound care for [a different resident].” Wearing gloves, the wound treatment nurse “removed the old dressing and cleanse the wound per physician’s orders […and] then changed her gloves without washing her hands, followed by measuring the wound.”

    The wound treatment nurse then changed gloves again without washing her hands, then proceeded to place the new dressing on the wound.” Upon interview, the Wound Treatment Nurse stated “Oh, yeah, I should have washed my hands, even though I was wearing gloves.” The actions by the nursing staff failed to follow the facility’s 03/31/2013 policy titled: Non-Sterile Dressing Change that reads in part:

    “Remove gloves, wash hands and apply new gloves.”

    In a separate summary statement of deficiencies dated 09/11/2015, the state investigator noted the facility’s failure to “follow standard infection control practices with regards to handwashing/hygiene, glove changing and sanitation of glucometer machines, during provisions of care.
