  • Failure to Provide a Resident Cares and Services in a Timely Manner to Ensure Their Highest Well-Being Was Maintained In a summary statement of deficiencies dated 11/06/2015, a state investigator noted the facility’s failure to “ensure a resident received the necessary care and services to prevent a delay in treatment for a resident with right-sided weakness, and bruising and swelling to the right arm.”The investigator also noted the facility’s failure “to ensure a resident with wounds received physician prescribed nutritional supplements, [and a failure to] have a plan to reduce moisture and promote wound healing for a resident with a moisture-associated wound.” The deficient practices by the nursing staff affected two residents at the facility “reviewed for necessary care and services.”

    In one case, the state investigator had findings after reviewing a resident’s 10/01/2015 Physician Order Sheet (POS) revealing the resident was diagnosed with Dementia with impaired safety awareness, delirium, fracture of the right metacarpal, Parkinson’s and dementia with poor trunk control and poor safety awareness.”

    The resident’s 03/19/2015 MDS (Minimum Data Set) revealed that the resident “is independent with bed mobility, transfers, ambulation, and walking.” However, the resident’s 10/09/2015 MDS revealed that the resident “is cognitively impaired and requires extensive staff assistance with transfers, dressing, hygiene, bathing, and toileting.”

    An observation of the resident was made at 8:05 AM on 11/03/2015 when the resident “was sitting in a low wheelchair with a plastic lap tray, using his feet to propel himself in the hallway.” At that time, the resident’s “right arm was tucked toward his body, resting on the lap tray.”

    The resident’s 07/22/2015, 10:00 AM Nurse Notes revealed that the resident “presents with right-sided weakness, the right arm appears limp, the right hand was swollen, unable to make a fist, unable to grab onto the walker.” At that time, the resident complained “of pain when touched, bruises noted to fingers. Will not stand to transfer. Doctor notified. Will await response.”

    Nearly five and a half hours later, the facility “received a call from the medical doctor on call” who stated that the resident will be transferred to the emergency room for an evaluation if it is okay with the family. The resident was sent to the emergency room more than six hours “after the resident presented with right-sided weakness, limp arm, swollen hand and bruising.”

    The resident’s Hospital Emergency Room documentation revealed a diagnosis of Renal Insufficiency, Dementia, Possible Cerebrovascular Accident – Stroke, enclosed fracture of the metacarpal bone.” A Licensed Practical Nurse stated on 11/04/2015 that the nurse “should use nursing judgment to determine if a resident should be sent to the hospital before obtaining a physician order.”

    The Administrator stated that based on the change in the resident’s condition and symptoms, “the nurse should have used nursing judgment and sent [the resident] to the hospital and notified the physician later.”
