  • Failure to Notify a Resident’s Physician of a Change in Their Condition Including a Decline in Their Health In a summary statement of deficiencies dated 07/14/2016, a formal complaint against the facility was opened by a state investigator for its failure to “follow its own policy on a change of condition for [a resident at the facility] reviewed for a change in their condition.” The deficient practice by the nursing staff involved one resident diagnosed with “bipolar disorder, schizophrenia, hypertension, heart disease and gastroesophageal reflux disease [GERD].” In addition, the resident was diagnosed as having “little or no interest in doing things, feeling down, depressed or hopeless.”The Psychiatric Rehabilitation Services Coordinator stated on the afternoon of 07/08/2016 that the resident was “feeling down after leaving his other nursing home.” A Licensed Practical Nurse providing the resident care that morning stated that the resident “was a loner […and] stated in his room […and] told her he did not know anyone in the facility that he knew from his previous nursing home.”

    The LPN also stated that the resident “paced all the time.” The Psychiatric Rehabilitation Services Coordinator stated that “the intervention that she did was to call [the resident’s] mother […and] stated that [the LPN] never told her that [the resident] expressed feelings of depression […and] stated that she would have charted and given [the resident] medicine.”

    The state investigator noted that the actions of the Psychiatric Rehabilitation Services Coordinator failed to follow the facility’s 06/26/2011 Resident’s Condition or Status Policy that reads in part:

    “To ensure that the resident’s attending physician and representative is notified of a change in the resident’s condition and/or status. The nurse will notify the resident’s attending physician when there is a significant change in the resident’s physical, mental or psychosocial status.”

  • Failure to Provide Every Resident a Safe Environment Free of Accident Hazards In a summary statement of deficiencies dated 07/14/2016, a state investigator noted the deficient practice of the nursing home’s failure to “ensure a resident received appropriate supervision.” The state investigator also noted the facility’s failure “to accurately identify suicide risk related to [the resident’s] history of paranoia / delusional suicidal ideation” involving a resident “who was recently hospitalized for delusional / paranoid suicidal ideation prior to the admission to the facility.”The state investigator reviewed the resident’s medical records to determine that the “40-year-old was admitted to the hospital through a referral after he was observed to be increasingly agitated; has been expressing aggressive behavior toward nursing staff and other residents.” The resident “was sent to another hospital due to threatening behavior.”
