  • Failure to Develop, Implement, and Enforce Policies Involving Abuse and Neglect at the Facility In a summary statement of deficiencies dated 01/22/2016, a state investigator noted the deficient practice of the staff while performing an annual licensure and certification survey concerning the nursing home’s failure to “follow their Abuse Prevention Policy for initiating resident background checks for criminal history record information within 24 hours of admission for [two residents at the facility] reviewed for criminal background checks.” This has the potential to affect all 62 residents in the facility.” The investigator noted that the facility’s 01/20/2014 Abuse Prevention Program Policy reads in part:“This facility shall check the criminal history background on any resident seeking admissions to the facility in order to identify previous criminal convictions.” This includes a “check for the resident’s name on the Illinois sex offender registry website and check for the resident’s name on the Illinois Department of Corrections sex registrant search page.”The facility’s Social Service Director stated on the morning of 01/22/2016 that nine residents’ criminal background checks were not initiated within 24 hours of resident’s admissions to the facility.”
  • Failure to Follow Procedures and Protocols to Eliminate the Spread of Infection In a summary statement of deficiencies dated 01/22/2016, a notation was made by a state investigator while performing an annual licensure and certification survey involving the nursing home’s failure to “practice good hand hygiene during resident care.”The deficient practice was first noted in the state investigator’s findings after an observation was made of two Certified Nursing Assistants performing incontinence care on a resident just after noon on 01/19/2016. “With gloved hands, [one CNA] cleansed [the resident’s] perineal area after [the resident] voided in the toilet. With the same soiled gloves, [the CNA] pulled [the resident’s] incontinent brief and pants up, adjusted [the resident’s] shirt, assisted [the resident] to pivot and sit in the wheelchair […and] then unbuckled [the resident’s] gait belt and removed [their (the CNA’s)] gloves.”The state investigator noted that the facility’s 04/15/2013 Policy titled Hand Hygiene under the category Purpose reads in part:

    Hand Hygiene is the single most efficient means of preventing the spread of infection. Indications for Glove Use: Gloves will be used for all cares following universal precautions […and] will be used for any care of the resident involving blood and bodily fluids.”
