  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Avoid Injury In a summary statement of deficiencies dated 06/20/2016, a state surveying agency opened a formal complaint against the nursing home for its failure to ensure a resident “was wearing her prescribed knee brace while being transferred.” The failure involved a resident at the facility whose kneecap was separated at the facility.A family member of the resident stated on 06/20/2016 that the resident “had a doctor’s appointment on 06/09/2016 and [that the resident] was told that her kneecap was separated and would have to have surgery again to repair it. The facility’s investigation into when the kneecap separated took place on 06/09/2016 after [the resident’s] surgeon contacted them and informed the facility about the new injury.

    The facility’s 06/09/2016 Investigation documents that a Certified Nursing Assistant took the resident’s brace off the resident’s leg “then transferred [the resident], causing [the resident] to ambulate a few steps, from the wheelchair to the shower chair.” The resident stated that “during the transfer, [the resident] heard [their] knee pop when [the resident] bent it and did not really think anything about it because knees pop.” The documentation revealed that the resident experienced “some pain.”

    The investigator interviewed the facility Assistant Director of Nursing on the afternoon of 06/20/2016 stated that the Certified Nursing Assistant “told her that [they] took the knee brace off of [the resident] to get [the resident] into the shower and did not check in the computer for instructions [… but] asked the nurse that was taking care of [the resident], which was [a Registered Nurse].”

    The Registered Nurse confirmed “that she told [the CNA] that the resident could have a shower and that [the CNA] could take off [the resident’s] knee brace for the shower but stated she never told [the CNA that] the resident could transfer without a knee brace. A review of the facility’ is 05/23/2016 CNA Report Sheet documents that the resident’s “knee immobilizer should be on when out of bed and while walking.”

  • Failure to Develop, Implement, and Enforce Policies Involving Abuse In a summary statement of deficiencies dated 03/26/2015, a notation was made by a state investigator while performing an annual licensure and certification survey involving the nursing home’s failure to “operationalize its Abuse Prevention and Prohibition Policy for screening new employees by failing to complete the required fingerprint background check upon a new hire of one housekeeper.”The state investigator noted that “this failure has the potential to affect all 72 residents residing in the facility.” A review was made of a personnel file for a facility Healthcare Employee hired on 02/23/2015 as “a full-time housekeeper. The investigator noted that the report “did not have any documentation of a Fingerprint Based Fee Applicant Background Check” as required by law.
