One incident was revealed to the state investigator involving a 9:15 AM 12/08/2015 incident where two Certified Nursing Assistants (CNAs) were transferring a resident “from the wheelchair to the bed using a mechanical lift.” At that time, one CNA “hooked a urinary drainage bag on the mechanical lift where the sling attached” before transferring the resident to the bed. The state investigator interviewed the facility’s Administrator who stated: “She would not expect this practice to be used when transferring residents who have an indwelling urinary catheter.”
The investigator reviewed the resident’s 04/15/2015 Care Plan revised on 11/11/2015 the documents of the resident “is at high risk for urinary tract infections due to [their medical condition including bladder problems] and history of urinary tract infections. Interventions listed on [the resident’s] Care Plan document to ensure catheter tubing and drainage bag are properly positioned to prevent urinary backflow or contamination.”
- Failure to Develop, Implement, and Enforce a Program That Investigates, Controls, and Keeps Infection from Spreading throughout the Facility In a summary statement of deficiencies dated 12/11/2015, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility’s failure to “remove contaminated gloves, and wash hands before touching multiple surfaces.”
This deficient practice by the nursing staff at Meadow Manor Skilled Nursing and Rehabilitation Center was first noted upon observation of two Certified Nursing Aides transferring a resident to the toilet at 11:00 AM on 12/08/2015. At that time one Certified Nursing Assistant confirmed that the resident “had been incontinent of urine” and had “a bowel movement in the commode.”
While providing care, both Certified Nursing Aides assisted the resident “to a standing position [while one Certified Nursing Assistant wiped the resident’s] rectal area with toilet paper.” While providing care, the Certified Nursing Assistant “touched multiple surfaces including the gait belt, wheelchair, bed and [the resident’s] clothing with the same gloves used to wipe [the resident].” In an interview with the state investigator, the CNA confirmed that “she did not remove her gloves and wash her hands before touching other surfaces in the room.”
