This failure resulted in IDPH [Illinois Department of Public Health] not being notified about the incident of verbal abuse within 24 hours.”

    In a third summary statement of deficiencies dated 04/22/2013, a complaint investigation is opened against the facility for its failure “to investigate for injuries of unknown origin for [a resident] and failed to immediately report an alleged violation involving staff to resident abuse to the state survey and certification agency; the facility also failed to complete a timely and thorough investigation for suspected staff to resident abuse for [a resident].”

  • Failure to provide residents an environment free of the spread of infection:
  • In a summary statement of deficiencies dated 01/23/2013, a notation is made of the facility’s failure “to follow its policy on contact isolation precautions for staff identification of a resident on contact isolation and the wearing of personal protective equipment (PPE) during the care and service of residents on contact isolation.” This is in response to a CNA providing care to a resident “on contact precaution. There was no personal protective equipment (PPE) in the immediate area. [The CNA] walked into the room without gown and gloves leaned on the bed placed her arm around [the resident] shoulder’s and repositioned [the resident]. [The CNA] then walked out of the room.” This is in direct violation of facility policy concerning spreading infections.
