In a summary statement of deficiencies dated 01/06/2016, a state surveyor made a notation during an annual licensure and certification survey concerning the facility’s failure to “implement prevention measures for pressure ulcers [bedsores; decubitus ulcer; pressure sores].” In addition, the state investigator noted the facility’s failure “to apply a dressing as ordered […and a failure to] identify risk for infections. The deficient practice by the nursing staff at Libertyville Manor Extended Care affected one resident at the facility.
The investigation involved a resident’s physician’s order indicating that the bedsore dressing needed to be “changed every three days and as needed.” The order involved the wound on the resident’s “right buttocks to cover with a dry dressing. Change daily and as needed.”
The state investigator noted that on 01/05/2016, the resident “was in bed sleeping on her left side with a pillow between her legs. Her wheelchair had a non-skid liner on the seat and no pressure redistribution cushion.” A Registered Nurse providing the resident’s care changed the resident’s dressings and noted that the resident “had a small pink open area to her right buttock. She had a non-stick pad with [medication] over the pressure ulcer […and an additional] pressure ulcer to the right outer ankle. The wound was red around the edges was slough [dead tissue] on the wound bed.
On that same day at 1:00 PM, the nurse stated “he spoke with the doctor was given orders to do a wound culture to the right outer ankle pressure ulcer to have an x-ray done to rule out [bone infection].” When the Registered Nurse was asked early in the day “about a wheelchair cushion for [the resident], she stated she did not know why there was not one there and that [the resident] slid out of the wheelchair. That is why there is a non-skid liner in the wheelchair.”
By the end of December 2016, the resident’s Nurse’s Notes “showed she has an unstageable pressure ulcer on her right outer ankle with a white base” noting that the pressure ulcer had become significantly worse to a possible life-threatening condition.
- Failure to Ensure That Residents Requiring Feeding Tubes Are Given Proper Treatment to Help Restore Eating Skills, If Possible
In a summary statement of deficiencies dated 01/06/2016, a state surveyor made a notation during an annual licensure and certification survey concerning the facility’s failure to “ensure a resident’s feeding tube was checked for placement prior to the administration of water flushes, medications, or giving enteral feeding.”
The investigator noted that a Registered Nurse at 4:00 PM on 01/03/2016 flush the resident’s gastronomy tube device with water “and then gave him his evening medication [… before continuing] to give his enteral feeding.” While the Registered Nurse “was giving medications at enteral feeding, [the resident] coughed three times.” Again standards of practice, the Registered Nurse “did not check placement prior to giving water flushes, medications or enteral feeding.”
