    In a summary statement of deficiencies dated 12/18/2015, a notation was made by the state surveyor concerning the facility’s failure to “change soiled gloves after toileting care of to residents reviewed for infection control.”

    The deficient practice was first noted in the state investigator’s findings after an observation of a resident was made at approximately noon on 12/15/2015. The resident “was assisted to the bathroom by a Social Services Aide and a Certified Nursing Aide.” After the resident urinated, the resident “was assisted to the standing position by [both employees].” With gloves on, the Certified Nursing Aide wiped the resident’s “peri-area with toilet paper and threw it into the toilet […and then] pulled up the resident’s pants, assisted [the resident] to the seated position in the wheelchair and reposition [the resident] in the wheelchair while wearing the same gloves used to wipe [the resident’s] peri-area.”

    A second observation was made of a different resident transferring from the wheelchair to the toilet with the assistance of two Certified Nursing Assistants just after noon on 12/15/2015. Both CNAs left the resident “on the toilet, unsupervised. At 12:39 PM, [both Certified Nursing Aides] entered [the resident’s] bathroom, gloved, stood [the resident] up and [one CNA] wiped [the resident’s] anus/perineal area from front to back […and] did not change her gloves or wash her hands after waking [the resident] and then touching [the resident’s] transfer device, clothing, bare skin and wheelchair with her soiled gloves.”

    The investigator noted that the deficient practice by the Certified Nursing Aides and Social Services Aide failed to follow the facility’s April 2012 policy titled: Handwashing / Hand Hygiene Policy and Procedure that reads in part:

    “This facility considers hand hygiene the primary means to prevent the spread of infection.”

    In a separate summary statement of deficiencies dated 01/29/2015, the investigator noted the facility’s failure to “prevent the spread of potential infection by following isolation precautions performing hand hygiene.” The deficient practice involved four residents at the facility “reviewed for infection control.”

    The deficient practice was first noted in the state investigator’s findings after reviewing a resident’s 11/14/2014 MDS (Minimum Data Set) documenting that the resident “requires the extensive assistance of one staff for ADL (activities of daily living) […and] has a urinary catheter in place.”

    The resident was diagnosed with VRE (Vancomycin-Resistant Enterococci) and MRSA (Methicillin-Resistant Staphylococcus Aureus), two highly contagious infectious diseases. The resident’s 11/19/2014 Care Plan directs the staff to “follow contact isolation at all times.” However, during an observation tour, the investigator noted that “at no time did [two employees] wear a gown while changing [the highly contagious resident’s] dressings.”
