Crystal Lake Nursing Home Resident Safety Concerns

  • Failure to Provide a Resident the Minimum Standard of Care to Treat an Existing Wound In a summary statement of deficiencies dated 05/31/2016, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility’s failure to “provide the necessary wound care by ensuring nursing staff routinely assessed a resident’s surgical incision and ensuring the resident received prompt intervention for surgical wounds showing signs of infection.” The state investigator noted that the failure by the facility involved one resident “reviewed for [receiving] improper nursing care.”

    The deficient practice by the nursing staff at Crystal Pines Rehabilitation and Health Care Center involved a resident whose 02/16/2016 Nurse’s Notes stated “right shoulder incision with staples intact. Edges well approximated. Hematoma at the distal end.” The Nurse’s Notes in the following days did not show a description of the wound. The 02/24/2016 through 02/28/2016 Nurse’s Notes all show “surgical incision to her right shoulder with a hematoma to distal end”. There is no documented assessment of the wound characteristics [including color, temperature, tissue type or drainage] documented on these dates.

    On 02/29/2016, the resident returned to the MD office for a visit with their physician. The Registered Nurse at the facility called Crystal Pines Rehabilitation to say that the resident would be “readmitted to (Hospital) for directed mission to have an incision and drainage of the right shoulder.” In addition to treatment, the resident received intravenous antibiotics. The state inspector noted that the deficient practice by the nursing staff failed to follow the facility’s August 2015 Wound Assessment Policy that reads in part:

    “It is the policy of the facility to assess each wound initially either at the time of admission or at the time the wound is identified. Each wound will be identified weekly thereafter or with any significantly noted change in the wound.”

  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Injury In a summary statement of deficiencies dated 05/03/2016, a state surveyor made a notation during an annual licensure and certification survey concerning the facility’s failure to “transfer [the resident] in a safe manner. This failure resulted in [the resident] having to be lowered to the floor, sustaining a fracture to her right femur.”

    The state investigator noted that upon review of the resident’s 04/07/2016 Progress Notes it was revealed that a Registered Nurse documented that a Certified Nursing Assistant (CNA) lowered the resident “to the floor during a transfer from the bed to the chair […and laid the resident] on her right side.”

    The Certified Nursing Assistant then assisted the resident “into a padded reclining chair.” At that time, the CNA assisted the resident “to the chair before the CNA made [the Registered Nurse] aware of the incident.” This action by the Certified Nursing Assistant violated established protocols after a fall including:

    “The protocol for this facility is no resident should be moved without the nurse assessing the resident first.”
