The Illinois Department of Health and other private health inspection firms conducted a survey on the level of health care patients receive from the staff and personnel of the health center. The survey brought to the surface, cases of poor patient supervision and abuse which could be construed as nursing home negligence. - Failure to notify physician of significant changes in a resident’s status: “Based on record review, staff/physician interviews, the facility neglected to provide care and services for R6, ensuring: 1) R6’s physician was made aware of significant and abnormal lab value used for monitoring [MEDICATION NAME] levels [MEDICATION NAME] time and International Normalized Ratio (PT/INR) in a prompt time frame 2)continuous monitoring of resident (R6’s) medical condition with abnormal PT/INR values for adverse effects of [MEDICATION NAME] 3)R6’s physician/alternate or medical director was notified immediately of R6’s onset of dark brown stools which required immediate medical intervention. These failures resulted in R6 having a massive [MEDICAL CONDITION] (GI Bleed), causing cardiopulmonary arrest, brain death and subsequently taken off life support 3 days after transfer to hospital.”
- Failure to make call lights accessible to residents: “On 9-20-2011 at 10:23AM R5 was lying in bed trying to reach the call light which was attached to the wall and R5 could not reach it. R5 stated at this time “My call light is broken, when you use it once you cannot use it again.”
- Failure to provide timely skin care for residents: “During wound treatment observation with E9 (Treatment Nurse) on 9/22/11 at 2:30pm, E9 completed the treatment to R11’s Sacrum and Scrotum and proceeded to position R11 for comfort. Surveyor requested of E9 to evaluate the plantar area of the right foot. E9 told surveyor that R11 has very dry, scaly skin on both of R11’s feet and pointed out the evidence of flaked off skin on R11’s bed linen. E9 stated that R11 received a bed bath earlier in the day and that no one reported any skin breakdown to her. E9 measured the area (.5 X 1.3 centimeters), applied Triple Antibiotic Ointment and covered it with a dry dressing. R11’s physician’s order [REDACTED].”
- Failure to administer medication in a timely manner: “R35 was not given [MEDICATION NAME] Regular Insulin for blood glucose of 302. Review of R35’s physician order [REDACTED].E16 had no comments as to why she did not administer R35’s her insulin. E3 (Assistance Director of Nursing) stated on 9-21-11 at 5:30PM, ” there is no reason as to why R35 had to wait over an hour to receive her ordered insulin in a timely manner for her abnormal glucose levels.”
