  • Failure to Investigative Report Allegations Involving Abuse In a summary statement of deficiencies dated 02/05/2016, an Illinois state agency investigator made a notation of the facility’s failure to “assure that a resident is free from any type of abuse, [a failure to] follow their policy on Abuse Prevention and [a failure to] investigate an allegation of verbal abuse involving staff members. These failures affect two [residents at the facility].”The deficient practice was first noted in the state investigator’s findings after reviewing in 08/13/2015 Face Sheet and Social Service Note and an interview with the resident in February 2016 who “was observed lying in his bed”. During the interview, the resident stated he had a “bad back” and continued to tell the investigator that they [the staff] were “aware of an order for pain medication that he can ask for every six hours if needed.”The resident stated that “when pain medication is needed, the nursing staff would not give it to him.” The resident claimed that “when asked for pain medication, both nurses call him names that include ‘drug addict and drug seeker’.” The resident also stated that the nurses “smart talk and sing ‘I like dope (illegal drug) songs’” that make the resident “upset and feel abused.”The state investigator noted that per the review “of the abuse investigations presented by the facility, there is no evidence that the facility investigated the allegations of verbal abuse reported by [the resident].” The facility’s Administrator stated “we will be conducting our investigation.”
  • Failure to Ensure Residents’ Drug Regimens Are Free from Unnecessary Medications In a summary statement of deficiencies dated 02/05/2016, the state agency surveyor noted the facility’s deficient practice to “create a symptomatic psychotropic reduction plan.” The investigator also claimed that the facility failed to “follow a Care Plan intervention and failed to complete an assessment to identify the presence of complications of an antipsychotic medication.”The investigator noted that the physician ordered the resident to receive antipsychotic medications “every night at bedtime.” The resident’s Care Plan documents an abnormal involuntary movement scale assessment would be performed every six months. However, “there is no evidence that [the assessment] was completed during the resident stay at the facility […and] there is no evidence that a systematic reduction plan was created for the resident’s use of [the antipsychotic medication].”The facility’s Minimum Data Set Coordinator verified on 02/03/2016 “that the psychiatrist determines the plan for psychotropic reduction […and] validated an absence of evidence of systematic [antipsychotic medication] reduction plan after going through the pages of the medical chart.”
