- Failure to Ensure Residents Remain Free of Accident Hazards and Provide Adequate Supervision to Avoid Injury In a summary statement of deficiencies dated 02/23/2015, a state surveying agency opened a formal incident investigation involving the nursing home for its failure to “revise a resident’s care plan and outlining a plan for supervision and monitor self-harming behaviors for one resident reviewed for behavior.” The deficient practice involved a 33-year-old male resident diagnosed with schizoaffective disorder with a history of physical aggression, substance abuse, and self-harm.” The resident “is also an identified offender assessed by the facility on 08/15/2014 to be a low risk.”The state investigator reviewed the resident’s Clinical Record revealing the resident “had a treatment plan for physical aggression, substance abuse, attempts to self-harm and for being an identified offender. It stated no incidences of self-harm.”The facility’s 9:00 AM 02/06/2015 Incident Report revealed that the resident “was standing in the hallway and cut his wrist with a small razor (disposable razor).” At that time, a Certified Nursing aide (CNA) immediately intervene and called a “code yellow and the razor was confiscated.” The resident “was assessed by [a nurse] to have cut 7.0 centimeters long across the left wrist. First aid was administered. The psychiatrist and physician were notified and the resident was sent to the hospital.”
The resident stated on the morning of 02/10/2015 that “he got the razor from the community when he was allowed to go on in the past. He but the disposable razor at the store and kept it hidden in his room for a couple of months.” The facility clinical Director stated on the afternoon of 02/17/2015 that the resident had “no depression and no suicidal thoughts prior to the 02/06/2015 incident.” The director also stated that the resident “was admitted to the facility with a history of self-harm and was in a treatment plan for self-harm… and was not supposed to have a razor in his possession.”
The Clinical Director also stated that “we conducted room checks and monitored [the resident’s] whereabouts while at the facility but never found a razor in his possession.” The facility’s Director of Nurses stated on the morning of 02/18/2016 that they had “talk to [the resident] after the 02/06/2015 incident.” The resident told the Director that “he got the razor from the community and kept it hidden in his room.” The Director did not know “where he hid the razor […and he] was not supposed to have a razor.”
A review of the resident’s revised 12/05/2014 Current Care Plan was conducted. The investigator noted that the Care Plan under the ‘history of self-harm’ does not have a plan on how staff members will monitor [the resident] for the use of unauthorized items and behaviors for self. The Care Plan (tubing plan) was not revised to alert staff members regarding this behavior […and the resident’s] self-harm incident of 02/26/2015 was not documented in the Care Plan.
The investigator requested to review the facility’s “policy or protocol for monitoring residents with self-harm behaviors.” However, no policies were presented during the investigation.
