A member of the Activities staff stated 11:15 AM on 06/10/2015, she was taking the minutes of the meeting and report the complaints and allegations to [the facility’s Activity Director] after the meeting.” As a result of that report, the activities staff member and the Activities Director “sent an email to [the facility’s Director of Nurses] on Friday, 06/05/2015 to inform her of the allegations made by [both resident’s].”
The investigator noted that the Director of Nursing “was asked about the allegations in the Resident Council Minutes on 06/05/2015.” The Director stated “receiving an email sent by [the Activities Director] on 06/05/2015, but did not open or read it until today 06/09/2015.” The Director was then “asked to review the facility’s investigation and the report sent to the Illinois Department of Public Health (IDPH).” In response, the Director of Nursing “stated an investigation often started and IDPH was not notified” in direct violation of state and federal nursing home regulations and laws.
- Failure to Provide Residents Proper Services to Prevent Urinary Tract InfectionsIn a summary statement of deficiencies dated 06/12/2015, the state investigator noted the facility’s failure “to assess, evaluate and implement a Bowel and Bladder program for residents identified as incontinent of Bowel and Bladder.” The investigator also noted the facility’s failure “to implement the Bowel and Bladder policy and procedure and to develop and implement a Plan of Care in order to improve or maintain their bowel and bladder function.”An interview with the facility’s Director of Nursing, it was revealed that “the facility has no Bowel or Bladder program, this area (Bowel and Bladder) is very new to us. We assigned the Minimum Data Set/Care Plan Coordinator to run the program, but I am the enforcer […and stated that] the facility does not have a certified restorative nurse with the knowledge necessary to manage a Bowel and Bladder program, nor are there any plans to obtain one.”
