  • Failure to Report Two Separate Harmful Incidences of Staff Treatment of Residents

    In a summary statement of deficiencies dated 12/08/2006, a surveyor noted the facility’s failure to “report two separate incidences require outside services to the Illinois Department of Public Health for [a resident].” This is in response to a resident falling “from her wheelchair and hit her head, receiving a lump on the left side of her head.” The injured resident “was sent to the hospital for x-rays after complaining of lower back and left hip pain. No incident report was found in the IDPH Regional Office.”
