This order permeated through the 200 and 300 Wings. The floors throughout the facility would dull.” The investigator also noted that “the mattress in room 307 [was] sunken in the middle and misshaped.”At 10:10 AM that same morning, a daughter of a resident at the facility spoke to the “300 Wing Housekeeper and asked “have you sprayed down here yet? The smell is pretty bad right now […and] the odor is very strong at this end of the building; some days are really bad.” The resident’s daughter also said that “the last three days have been bad. Soon as I say something to the housekeeping staff, they come in spray. They need to try and keep ahead of it.”At 11:00 AM that day, the resident stated that her “son and daughter buy her at least two cans of air freshener a month to use in her room. If a friend stops by unannounced and she has not sprayed her room, she is embarrassed by the odors.” The resident also indicated that “if she leaves the facility she sprays her clothing so [she] does not go out in public spelling like the facility.”
- Failure to Provide Treatment and Services to Prevent the Development of a Bedsore or Allow an Existing Bedsore to Heal In a summary statement of deficiencies dated 01/19/2016, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility’s failure to “complete document daily skin assessments, identify new open areas, complete risk assessments, develop and implement specific pressure ulcer prevention interventions, and provide treatment as ordered.” The deficient practice affected three residents at the facility.One failure involved a resident at the facility being observed at 9:50 AM on 01/14/2016 who “was transferred to the toilet” by two Certified Nursing Assistant who “perform perineal cleanser after [the resident] had a bowel movement.” At that time, the resident “cried out in pain each time [a Certified Nursing Assistant] wiped the area.” The facility’s Director of Nursing replace the resident’s “dressing to her right buttock. An irregular shape open area was observed just above [the resident’s] anus.” The facility’s Wound Nurse and the Director of Nursing “stated this was a new area and it had not been reported.”
