The resident’s Incident Report indicated that the patient “fell in the bathroom unattended and was sent to the local hospital shortly after due to complaints of back pain.”

The Director of Nursing said, “Most false can be prevented by educating and asking the resident to call for assistance if they are alert and are able to make their needs known. Everyone is at risk for falls.”

Failure to Provide and Implement an Infection Prevention and Control Program – Deficiency 

In a summary statement of deficiencies dated November 17, 2020, the state inspectors determined that Alden Estates of Evanston Rehabilitation Nursing Facility failed to “Identify rooms that would be utilized specifically for Kovalic -positive residents, failed to ensure that housekeeping personnel prevented cross-contamination between covert/non-covert residents and failed to ensure that housekeeping staff utilized disinfecting agents per manufacturer’s specifications.”

The State investigators conducted a tour of the 3rd floor where there “was no designated area for the covert positive residents.” Instead, the rooms were “Interspersed with rooms of quarantine residents that had no symptoms and rooms that were recovering/waiting to be transferred to the 2nd floor.”
