The Flow Sheet documents that R15 received her [MEDICATION NAME] at 12:20 PM on 8/25/09.On 8/27/09 at 12:20 PM E7 (Nurse) confirmed that R15 is to receive [MEDICATION NAME] prior to physical therapy. “
  • Failure to keep the rate of medication errors below 5%: “On 8/25/09 from 8:32 am to 8:50 am observed E5 (Nurse) administering the morning medications which included [MEDICATION NAME] 20 milligrams (mg) one tablet, [MEDICATION NAME] 10 mg one tablet, [MEDICATION NAME] Injection, Levetiracetam ([MEDICATION NAME]) 5 milliliter (ml), [MEDICATION NAME] 80 mg, and [MEDICATION NAME] (Insulin) 10 units to R25. E5 did not give [MEDICATION NAME] 25 mg half tablet and Chlorhex Glu Solution 15 ml.R25’s current Physician Order Sheet (POS) documents to administer [MEDICATION NAME] 25 mg half tablet daily at 9:00 am and Chlorhex Glu Solution 15 ml twice daily at 9:00 am and 9:00 pm.”
  • Failure to follow a program that controls and keeps infection from spreading: “On 9/16/11 at 9:35 AM, E3 (Certified Nursing Assistant-CNA), rendered incontinent care to R18. E3 removed the soiled incontinent pad, used wet wipes to clean, R18, then placed a clean incontinent pad underneath R18. E3 did not wash hands nor change gloves between the dirty task and to clean task. On 9/16/11 at 1:00 PM, E2 (Director of Nursing) stated, staff should do handwashing before and after resident contact and glove usage. We use gloves as part of personal protective equipment (PPE) and render standard care. We change gloves with a different task. R18 is on isolation precautions for
    [MEDICAL CONDITION].”
