This failure contributed to [the resident’s] pressure ulcer to her coccyx becoming infected on June 11, 2015.”
In a separate summary statement of deficiencies dated 05/13/2015, a complaint investigation was opened concerning the facility’s failure “to eliminate pressure to [a resident’s] foot. They failed to assess and document the wound characteristics for [the resident’s] pressure area.” This is in response to a resident who was admitted with a fourth and fifth-digit amputation and diabetes mellitus on the right foot noting a wound with chronic tissue that was improperly treated by the facility.
- Failure to Provide a Residential Environment Free of Accident Hazards:
In a summary statement of deficiencies dated 10/09/2014, a complaint investigation was opened concerning the facility’s failure “to provide supervision to prevent falls.” This is in response to three separate incidences occurring in October 2014. In one incident, a resident stated that “I fell in the bathroom I was left alone. I turned to wipe and fell off the toilet. I hit the left side of my face, left shoulder, and left hip.”
In another incident, a resident documented to be at an increased risk for falling was “observed in the bathroom by himself on the toilet.” An assistant administrator indicated that the resident “took himself to the bathroom.” The resident had stated that a month earlier, he had fallen when transferring “himself from the bed to the chair.”
