In a summary statement of deficiencies dated 11/10/2015, a complaint investigation is opened against the facility for its failure “to address residents need in a timely manner when the call light was activated for [2 residents].” This is in response to an observation where a resident’s “call light began alarming” at 6:40 AM on 11/7/2015. Four minutes later, an LPN (licensed practical nurse) and RN (registered nurse) “look down the hall at the call lights and continued to count narcotics on the medication chart.” It took 38 minutes for the LPN to answer the resident’s call light.
In a summary statement of deficiencies dated 10/02/2015, complaint investigation was opened against the facility for its violation of failing to “immediately report an allegation of sexual abuse to the state surveyor and certification agency for [a resident].” This is in response to an allegation that an “unknown male certified nursing assistant (CNA) grabbed [the resident’s] breast and kissed [the resident] good night on the cheek.”
