Concerning Findings Regarding Patient Care At Central Nursing & Rehabilitation Center

According to data from nursing home surveys conducted by the Illinois Department of Health other agencies, there have been numerous episodes of poor patient care at Central Nursing & Rehabilitation Center which can be construed as nursing home negligence. Our nursing home attorneys reviewed data compiled by ProPublica and found the following conditions that can be construed as nursing home negligence:

  • Failure to investigate allegations of abuse between residents:  “E1, (director of nursing) was interviewed and stated, “R12 and R30 had an altercation, and went to the hospital. We did an investigation and sent the incident reports to IDPH.”
    The review of the abuse incident reports received was confirmed with E1 as being submitted to IDPH, and did not contain the interview of the witness. In addition, R12 and R30’s medical records did not denote a follow-up by social services to determine residents feelings. R12 was unavailable for an interview, and R30 was still hospitalized during the onsite visit of 8/15-8/18/2011.”
  • Failure to keep the rate of medication errors below 5%: “On 8/16/11 at 7:58 am, the surveyor approached E12 (Licensed Practical Nurse, LPN) to watch the medication pass. E12 stated that E12 had already completed the medication pass for the entire unit. The facility’s Medication Pass policy documents the following times for medication pass: 9:00 am, 1:00 pm, 5:00 pm and 9:00 pm. E1 (Director of Nursing, DON) stated the nurses may start the medication pass 1 hour prior to the scheduled time, which would be 8:00 am (to start) for the morning medication pass. E12 stated she had completed the entire medication pass prior 8:00 am. When asked if R32 was out of the facility during the scheduled medication time, E12 stated “no”. The physician’s order [REDACTED].”
  • Failure to provide incontinence care in a timely manner: “On 8/15/11 at 10:12am, during the initial tour a pervasive odor was noted coming from the room of R1. Upon entering the room, R1 was noted sitting in the wheel chair wearing pants that were saturated with urine. R1 stated, she has incontinent episodes and requires staff assistance for care. E7, (CNA, Certified Nursing Assistant) entered the room and stated, “I’m getting ready to give her a shower.” At 10:28 am (16 minutes later), R1 was still sitting in the chair, wearing the same pair of urine soaked pants. There was no staff in the room at this time. Upon inquiry of R1’s condition at this time, E7 stated, “I only have 2 hands.” R1 remained in the same wet pair of pants and did not receive incontinence care or shower prior to 10:39 am (27 minutes after the initial observation).”
