[The resident] stated that staff are causing the bruises on his hands.” Documentation of the time indicated that “there is nothing in the investigation report that shows other residents or direct care staff were interviewed.”
- Failure to Provide a Residential Environment Free of Physical Restraints
In a summary statement of deficiencies dated 10/31/2014, a complaint investigation was opened indicating that the facility “failed to assess and document the risks versus benefits for use of restraints [on a resident].” This is in response to a resident being observed “to have a seatbelt like restraint across her lap on her wheelchair.” At the time of the report, there was no restraint assessment in [the resident’s] medical records regarding [the resident’s] seat belt, her inability to remove the seatbelt, and the risks versus benefits of using the seatbelt.”
