[The resident] said she reported the incident to several staff members including [the restorative aid] and [the guest relations officer].”
In a summary statement of deficiencies dated 06/16/2015, a complaint investigation was opened against the facility for its failure “to develop an accurate care plan that addresses wandering status of [the resident] reviewed for comprehensive care plans.” This is in response to a review of a resident’s documented wandering behavior that was recorded on 5.30.15 indicating “wandering behavior occurred 1 to 3 days.” Progress notes throughout the month indicated that the resident refused “to stay in bed, had to place a wheelchair at the nurse’s station and as a noncompliant, tried to get on the elevator, and into other resident’s rooms, needs to be watch at all times and is a danger to himself.”
