Bridgeway Senior Living Ratings & Violations

Making mistakes when administering medications is a serious problem and health concern to nursing home residents throughout the state of Illinois. These errors are often caused by a lack of supervision and training by unqualified nursing personnel. In fact, the Bensenville elder abuse attorneys at Rosenfeld Injury Lawyers LLC have represented many nursing home residents who have been victimized by the neglectful actions of their caregivers.

For residents in nursing facilities, any medication error can produce dire consequences. In some incidences, the resident is given the correct drug at the wrong dose, the wrong medication, or a drug that should have been given to another resident. While minor discrepancy might go unnoticed, taking the wrong medication or at the wrong dosage could be catastrophic and produce serious injuries or death.

The most common types of medication mistakes in nursing facilities involve:

  • Skipping a dose
  • Combining the wrong medications
  • Administering expired products
  • Administering the drug with or without consuming food in direct opposition to directions
  • Administering the drug at the wrong time of day
  • Administering the drug through the wrong route (oral, suppository, feeding tube)
  • Failing to monitor the resident’s reaction to a drug
  • Failing to properly report any side effect and provide necessary treatment

Bensenville Nursing Home Resident Safety Concerns

On an annual basis, federal and state nursing home regulatory agencies routinely update their comparative nursing home database information and post the results on their website portal Medicare.gov. Many families use this valuable information as an effective comparative tool to decide which location in their community best serves the needs of their residents to ensure that their health and hygiene needs are met on a consistent basis.

Currently, Bridgeway Senior Living maintains an overall one out of five available star rating compared to other nursing homes nationwide. This includes three out of five stars for quality measures, two out of five stars for health concerns and one out of five stars for staffing issues.

Our Bensenville nursing home neglect attorneys have found numerous opened investigations, filed complaints, health violations and safety concerns involving this facility. Two of these concerns include:

  • Failure to Ensure That Every Resident’s Drug Regimen Is Free from Unnecessary Medications

In a summary statement of deficiencies dated 11/20/2015, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility's failure to "identify specific target behavior symptoms for the use of anti-psychotic medications, attempt to initiate non-pharmacological interventions prior to the use of anti-psychotic medications, monitor the effectiveness of the medication on resident behaviors and develop resident specific plans for drug reduction.”

The deficient practice by the nursing staff at Bridgeway Senior Living applied to four residents at the facility “evaluated for the use of anti-psychotic medications.” In one incident, a resident was admitted to the facility with “a behavioral assessment completed noting [that the resident] becomes restless and agitated related to cognitive loss which occurs toward the evening.” At that time, the resident “believes must find her husband and children. Interventions include reassurance, redirection, and medication.”

However, the state investigator noted that “there were no specific target behaviors for the use of [the antipsychotic medications]. The Care Plan does not include interventions from the behavioral assessment and the behavioral tracking does not list specific target behaviors for [the resident].”

  • Failure to Minimize Medication Error Rates Involving Administering the Wrong Drug, Wrong Dose or the Right Medication at the Wrong Time

In a summary statement of deficiencies dated 11/20/2015, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "ensure the medications were administered to residents as ordered by the physician. This failure resulted in the observation of 24 opportunities, with three errors occurring at an error rate of 12.5%.” The deficient practice by the nursing staff at Bridgeway Senior Living affected three residents observed during a pass of administering medications.

One incident occurring on 11/17/2015 at 11:41 AM involved administering eight medications scheduled for 9:00 AM to a resident upon a physician’s order. However, the nurse in charge of administering the medications “could not find this medication to administer” saying it is not available. At that time, the nurse “signed off this medication as given for 9:00 AM even though the medication was not given.”

Jonathan Rosenfeld was professionally objective, timely, and knowledgeable. Also, his advice was extremely effective regarding my case. In addition, Jonathan was understanding and patient pertaining to any of my questions or concerns. I was very happy with the end result and I highly recommend Jonathan Rosenfeld. Michonne Proulx
★★★★★
Extremely impressed with this law firm. They took control of a bad motorcycle crash that left my uncle seriously injured. Without any guarantee of a financial recovery, they went out and hired accident investigators and engineers to help prove how the accident happened. I am grateful that they worked on a contingency fee basis as there was no way we could have paid for these services on our own. Ethan Armstrong
★★★★★
This lawyer really helped me get compensation for my motorcycle accident case. I know there is no way that I could have gotten anywhere near the amount that Mr. Rosenfeld was able to get to settle my case. Thank you. Daniel Kaim

Jonathan did a great job helping my family navigate through a lengthy lawsuit involving my grandmother's death in a nursing home. Through every step of the case, Jonathan kept my family informed of the progression of the case. Although our case eventually settled at a mediation, I really was impressed at how well prepared Jonathan was to take the case to trial. Lisa
