In a summary statement of deficiencies dated 11/20/2015, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility's failure to "identify specific target behavior symptoms for the use of anti-psychotic medications, attempt to initiate non-pharmacological interventions prior to the use of anti-psychotic medications, monitor the effectiveness of the medication on resident behaviors and develop resident specific plans for drug reduction.”
The deficient practice by the nursing staff at Bridgeway Senior Living applied to four residents at the facility “evaluated for the use of anti-psychotic medications.” In one incident, a resident was admitted to the facility with “a behavioral assessment completed noting [that the resident] becomes restless and agitated related to cognitive loss which occurs toward the evening.” At that time, the resident “believes must find her husband and children. Interventions include reassurance, redirection, and medication.”
However, the state investigator noted that “there were no specific target behaviors for the use of [the antipsychotic medications]. The Care Plan does not include interventions from the behavioral assessment and the behavioral tracking does not list specific target behaviors for [the resident].”
- Failure to Minimize Medication Error Rates Involving Administering the Wrong Drug, Wrong Dose or the Right Medication at the Wrong Time
In a summary statement of deficiencies dated 11/20/2015, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "ensure the medications were administered to residents as ordered by the physician. This failure resulted in the observation of 24 opportunities, with three errors occurring at an error rate of 12.5%.” The deficient practice by the nursing staff at Bridgeway Senior Living affected three residents observed during a pass of administering medications.
One incident occurring on 11/17/2015 at 11:41 AM involved administering eight medications scheduled for 9:00 AM to a resident upon a physician’s order. However, the nurse in charge of administering the medications “could not find this medication to administer” saying it is not available. At that time, the nurse “signed off this medication as given for 9:00 AM even though the medication was not given.”
