In a summary statement of deficiencies dated 1/16/2015, the facility failed “to ensure staff report injuries of unknown origin to the Administrator immediately for [a resident].” This is in response to the facility’s 1/8/15 Transmission Report indicating that a daughter of the resident “called the facility on 12/31 concerning [the resident’s] left hand at her medical appointment on the 12/30/14.” At that time, the resident “stated that the Nurse on duty tried to pull her off the bat by her hand but was unable to point out the Nurse who she believed caused the injury.”
As a part of the investigation, to a facility occupational therapist and physical therapists “noted the bruising on [the resident’s hand] and on 12/29/14. The report documents [the resident’s] has Dementia and could not find the Nurse.” Against policy, neither of the medical staff reported “the bruising on [the resident’s] hand immediately to [the Administrator].”
