- Failure to Report and Investigate Any Act or Allegation of Abuse, Neglect or Mistreatment of Residents
In a summary statement of deficiencies dated 03/03/2016, a complaint investigation against the facility was opened for its failure to "investigate an injury of unknown origin after staff found a resident, who is usually agitated, with red marks on his face.” The deficient practice by the nursing staff at Brentwood North Healthcare and Rehabilitation Center was first revealed by Certified Nursing Assistant (CNA) at the facility at 12:27 PM on 03/02/2015 stating that the resident’s “left cheek had pink marks on it. He [the CNA] reported to the nurse right away.”
That same day at 12:30 PM, the facility’s Director of Nursing stated that “the daughter [of the resident] had come to her office concerned about her dad [… stating] he had redness on his face and was agitated, which was not normal for him. She stated she went and talked with the CNA who looked at [the resident’s] face.” The Certified Nursing Assistant “called the daughter back in [to the room indicating that “it was just petechiae (non-raise, tiny red dots).”
However, a few days earlier on 02/27/2015 on a Saturday night, the Director of Nursing received “a call from the supervisor working at the facility saying that the Hospice Nurse for [the resident] called and said that the daughter was asking about how to report abuse to the police [… stating that since the resident] was already discharge once she got the phone call she did not do/start an abuse allegation [… stating] she already knew about the concern [redness on the face] that she did not treat it as abuse.” The state investigator noted that “no investigation was done or reported to the State agency” as required by law.
- Failure to Ensure Every Resident Is Provided an Environment Free of Accident Hazards and Risks and Provide Adequate Supervision to Prevent an Avoidable Accident
In a summary statement of deficiencies dated 07/01/2015, a complaint investigation was opened by a state surveyor against the facility for its failure to "provide specific fall intervention [...and a failure] to modify Care Plan interventions after multiple incidences of falls [...and a failure] to provide supervision to prevent further falls to the resident who was identified at high risk for falls.”
The failure by the nursing staff at Brentwood North Healthcare and Rehabilitation Center “resulted in [the resident] sustaining a large subdural hematoma after a fall on 06/21/2015, when left without supervision, which led to the resident’s documented decline in ADL (activities of daily living) and hospice care placement.”
The state investigator noted that the accident could have been prevented after reviewing the resident’s 02/24/2015 and 05/25/2015 MDS (Minimum Data Set) revealing that the resident “required supervision with a one-person physical assist for ambulation and eating.” The investigator also noted that “during the past few months, [the resident] sustain three falls (03/24/2015, 06/14/2015 06/21/2015).”
