The facility failed to keep chemicals in a locked storage, and failed to ensure resident safety by allowing a space heater in the residence room.” This negligent management of the facility jeopardized seven residents.
In another summary statement of deficiencies dated 02/10/2012, a complaint investigation was opened claiming that the facility had failed “to provide adequate monitoring prior to injury, failed to call a physician immediately after injury occurred, and failed to send resident to hospital emergency room after self injury.” This is in response to an incident involving a resident diagnosed with “major depression, bipolar with psychotic features, anxiety and a history of suicidal ideations and self injuries.” On 1/23/12, the resident complained to nursing staff that her “depression had worsened and she wanted to go to the drugstore, buy a razor and cut her stomach.” However, “no physician was notified of her suicidal ideations.”
On 1/25/12, the resident “went to the drugstore and bought herself some razor blades as she had threatened staff to do on 1/23/12 and [the resident] cut her abdomen. [The resident] was sent to the hospital on 1/25/12 after nursing staff informed the psychologists of [the resident’s] suicidal ideations and her infliction of self injury.”
