Disquieting Findings on the Patient Care At Balmoral Home
According to data from nursing home surveys conducted by the Illinois Department of Health other agencies, there have been numerous episodes of poor patient care at Balmoral Home which can be construed as nursing home negligence. - Failure to report allegations of abuse to the State Agency: “On 01/26/12 at 9:45 AM R21 reported to the surveyors that E11 (Certified Nurse Assistant) grabbed her by the arm, while taking her to the shower. R21 stated, “she was rough with me.” R 21 showed the surveyor her arm. R21 has a black and blue bruise to the left upper arm approximately 1 inch by 2 inches in size. R21 stated the incident occurred, “1-2 days ago” and reported it last evening, on 01/25/12, to the nurse on duty. R21 could not remember the nurses name, only the first letter of the name. E1(administrator) was notified about the incident on 1/ 26/12 at 10:07 AM at the daily status meeting. On 01/26/12 at 3:45 PM at the daily status meeting, E1 presented a completed investigation report. On 01/27/12 at 11:55AM E1 was asked if the incident was reported to the Illinois Department of Public Health (IDPH) after the incident was reported to the facility. E1 stated, “I thought I had 24 hours to report the incident.”
- Failure to develop complete care plans for residents: “Review of all reviewed residents current care plans does not identify a care plan related to high cholesterol.; its goals and approaches. Interview with E6(Registered Nurse) on 3-14-12 at 11:30 AM stated that the nurses monitor the residents for potential side effects and will notify the doctor as warranted. Interview with E3(Director of Nursing) on 3-14-12 at 1:30 PM stated that all residents with high cholesterol should have an individualized care plan.”
- Failure to take measures to prevent the development of pressure sores: “Review of the comprehensive care plan dating back to 03/02/11 showed no preventive pressure relieving measures to address avoiding pressure ulcers. The comprehensive care plan did not address complications that often put patients with [MEDICAL CONDITION] at higher risk for developing skin problems due to decreased sensation and decreased mobility. The initial reference to the then existing pressure wounds was on 11/30/11 related to pressure ulcer treatment.”
- Failure to keep the rate of medication errors below 5%: “Observed E13 check R32’s blood sugar with glucometer; results displayed 333. Interview with E13 on 1-25-12 at 11:25 AM, states R32 is suppose to get 8 units of insulin. Observed during medicine pass on 1-25-12 at 11:25 AM draw up in syringe 9 units of [MEDICATION NAME]for R32. Surveyor prompted E13 twice to discard one unit of insulin before it was given to R32. Observed during medicine pass on 1-25-12 at 12:46 PM E13 give medicine to R33 [MEDICATION NAME] 300 milligrams. Interview with E13 on 1-25-12 at 12:47 PM, states [MEDICATION NAME] is not available. E13 states she ordered [MEDICATION NAME] yesterday and this morning. E13 states she called the medicine twice to pharmacy but that you can fax the request also.”
