Concerning Findings Regarding Patient Care At Arbour Health Care Center
A survey conducted by the Illinois State Department of Health and other private agencies discovered numerous cases of poor patient supervision, inadequate care equipment and abuse which can be construed as nursing home negligence. - Failure to keep the rate of medication errors below 5%: “On 03/22/12 at 7:45 AM E5 (Licensed Practical Nurse) did not administer the medication [MEDICATION NAME] 40 milligrams to R23. E5 stated they ran out of the medication, but she “would order the medication from the pharmacy today”. On 03/23/12 at 10:00 AM the MAR shows that the [MEDICATION NAME] was not documented as given on 03/22/12.”
- Failure to give specialized rehabilitative services to address substance abuse: “On 10/12/11, social service notes also indicated that R12’s roommate complained that R12 was smoking crack. R12 denied it and R12 was told that on his next offense, a drug test will be done. There is no indication that R12’s physician was made aware of above to obtain orders for drug test and confirm current use of cannabis or crack cocaine. R12 even denied he smokes marijuana even though he has a criminal offense of cannabis possession and use. Also, R12 was not placed in any substance abuse program to address potential or actual active use of drugs.”
- Failure to give proper treatment to prevent bedsores: “R4 is completely dependent on staff for all activities of daily living, and was noted with contracture to right leg and right arm. R4 did not have a pressure relieving mattress or pillows between legs of the noted bony prominences. After prompting, a skin check R4 was observed with a [MEDICAL CONDITION]/redness/non-blanchable pressure sore to right sacral area. R4 heels were red, and did not have protectors on. E5 (certified nurse aide) was interviewed 5/3/11, and stated,”it was red ( referencing sacral) about 7:30am when I clean R4, I did not tell the nurse.” ”
- Failure to ensure that each resident’s drug regimen is free from unnecessary drugs: “On May 4, 2011, E3 (LPN) confirmed that [MEDICATION NAME] was given to R16. E2 (Director of Nurses) also confirmed that [MEDICATION NAME] was given to R16 on April 27, 2011 at 11:45 AM. On May 5, 2011, E1(Administrator) stated that physician had discontinued [MEDICATION NAME] and changed it to [MEDICATION NAME] because R16 had an allergy to [MEDICATION NAME].”
- Failure to provide services to attain the highest well being of all residents: “R15 indicated that on 3/16/10, the lab technician came to draw his blood. After drawing his blood, the technician called him by his roommate’s name to say goodbye to him and he realized that he wasn’t supposed to have a blood draw. Nurse’s note indicate incident occurred on 3/15/09 and was already identified as laboratory error incident. R15 did go to the doctor on 3/16/09 and was given an order of antibiotic Keflex for the redness and swelling on left arm. The antibiotic was not ordered from Pharmacy until 3/18/09 with E1 indicating that R15 did not give MD paperwork until 3/18/09. Nurse did not notified MD about the condition of R15 regarding the status of the site and not getting antibiotics that was ordered 2 days earlier. On 3/20/10 R15 was hospitalized due to [MEDICAL CONDITION] on left arm identified on E1’s documentation as still red and swollen with open lesion on left wrist area.”
