[The resident] was observed without heal protectors in place.”
In a summary statement of deficiencies dated 06/18/2014, a complaint investigation is opened against the facility for its failure “to provide restorative services for perineal and genital care and ACE bandage wraps for [2 residents at the facility].” This investigation is partly in response to a resident admitted to the facility with “stage I and stage II pressure sores.” The facility staff observed an LPN performing improper incontinence care for the resident and neglecting to use ACE wraps as directed by physician’s orders.
