  • Failure to screen employees on the health care workers registry: “ The following employees were hired and employed by the facility prior to the facility checking the employees status on the health care workers background registry: E15 (nurse) was hired by the facility on 2/27/12. E16 (laundry) was employed on 3/26/12, E17 (dietary) was hired on 2/20/12, and E18 (housekeeping) was hired on 3/6/12.”
  • Failure to identify and treat pressure sores: “On 5-10-2012 at 11:10AM R22 was sitting in her room in a wheelchair. E9(Certified Nursing Assistant) was passing by the room. R22 verbally alerted E9(CNA) that she needed assistance. E9, E10(CNA’S) both rendered incontinent care for R22 at this time. E9 and E10 took R22 from her wheelchair and placed her in the bed. R22’s incontinent brief was filled with loose liquid stool. After E9(CNA) cleaned away some of the loose liquid stool with soap and water, a partial thickness loss of skin about the size and width of a quarter was noted to R22’s left sacral area, there was no dressing on the opened skin area. E9(CNA) stated at 11:30AM, “This is a new sore, I just found out about it yesterday there is treatment for [DIAGNOSES REDACTED].” E9(CNA) applied Zinc Oxide Cream to the entire sacral area after cleaning the liquid stool with soap and water. On 5-10-2012 at 11:55AM, E11(Nurse) assigned to care for R22 stated, “Treatment orders are on the Physician Order Sheet, there are no treatments for R22.” On 5-10-2012 at 12:06PM, E8(Treatment Nurse) stated, “I am not aware that R22 has any treatment. I am the only treatment nurse for all 4 floors, I will go and assess R22 now.”
  • Failure to ensure the area is free from hazards and provide supervision to prevent avoidable accidents: “On 5/9/12 at 9:55 AM, R19’s call light was on and E4 (Dementia Coordinator) responded to the call light. E4 stated R19 stated he was awaiting to be transferred to the toilet. E4 got E13 ( nurse aide) to assist R19. R19 stated he is very ashamed that he soiled his brief. R19 stated that a nurse, who he does not know her name, brought him an adult brief in the toilet room and stated she would be back but never came back. R19 stated in the mean-time he soiled himself. E13 removed the adult brief and R19 had fecal matter all over his buttocks, between legs and frontal area. On 5/9/12 at 10:05 AM, E7 (nurse) confirmed she was the one who brought R19 the adult brief. She stated she left him alone to give him privacy and stated R19 can transfer himself. E7 stated if R19 soils himself, he can tell them. R19’s quarterly Minimum Data Set ((MDS) dated [DATE] documents R19 requires one person extensive assist. R19’s current care plan documents he requires staff assistance for transfers which was initiated 2/6/12 and it documents R19 is at high risk for falls (initiated 10/24/11) due to history of multiple falls. R19 has an unsteady gait, poor balance and poor safety awareness. ”
  • Failure to provide the adequate amount of protein in a meal: “On 5/10/12 at 11:50 am during the noon meal on the first floor, E23 (Dietary Aid) served 2 slices of the glazed baked ham to the residents who received a general diet. On 5/10/12 at 1:15 pm E24 (Cook) was asked to weigh 2 slices of glazed baked ham. Two slices of the glazed baked ham weighed 1.5 ounces. According to the facility’s Menu Guide Report for 05/10/12, the lunch portion of the glazed bake ham is 3 ounces. Each resident on a general diet received half of the required protein for that meal. This error calculates to be 1.50 ounces per resident, instead of the 3 ounces as planned for the meal. ”
