Incredulous Findings Concerning Patient Care at Alden Princeton Rehab & Healthcare Center

A survey conducted on the level of care provided to patients admitted into the Alden Princeton Rehab & Healthcare Center, by the Illinois Department of Health, showed that patients experienced periods of poor care and abuse during their stay.   • Failure to have fall prevention measures in place and provide proper care after a fall: “ R5 sustained an unwitnessed fall on 3/1/12 from which she complained of leg pain. Portable X-ray results were obtained 5 hours after the fall. R5 was transferred several times from chair to bed during the hours following the fall. R5 was evaluated in the emergency room 7 hours after fall occurrence. Hospital records shows a subcapital comminuted left [MEDICAL CONDITION] neck fracture with external rotation and superior displacement.”
  • Failure to promote healing and prevent pre-existing pressure ulcers from increasing in size: “When the top covers/linen were removed and R3 turned to the right side using the chuck under R3, the bottom sheet covering the mattress observed to have an extra large amount of semi-dry dark brown colored stool with an odor on it. The chuck had a moderate amount of the same stool on the top portion of it. In R3’s diaper was a large amount of semi-dry sticky (in appearance) stool in it. The large deep sacral pressure ulcer had no covering at all over it. At 10:55am, E4 (treatment nurse) joined the team in R3’s room. E4 ask if R3 should have a covering over the sacral wound? E4 stated, “There should be a dry dressing covering the wound.” Then E21 stated, “I (E21) didn’t know it was uncovered, but I (E21) do know R3 has a treatment to it (sacral pressure ulcer). I (E21) don’t know when the last time R3 was changed.”
  • Failure to report allegations of abuse to site supervisors: “R’s 23 was observed on 3/11/12 at 5:30pm screaming in the hallway, “I am tired of R35 coming to me asking for oral sex, when are you going to do something.” R23 was interviewed, and stated,”I am sick and tired of R35 asking me for oral sex; this has been going on since I have been here. This makes me feel unsafe. I told E17 (PRSC/case manager) a little while back, and E26 (security guard) a little while ago. R35 also do it to R22 and 27.” R23 was admitted to the facility 1/7/12 per facility face sheet. R22 interviewed on 3/11/12-5:40pm, and stated,”Yes, I am sick of R35, always ask me to give him oral sex. I told E17 (PRSC) about it. Going on since I came here. If R35 keeps it up, I am going to go mental, I am a marshal in the army.” R22 was admitted per face sheet on 2/6/12. R27 observed on 3/11/12 5:50pm in 3rd floor dayroom in wheelchair, bilateral [MEDICAL CONDITION]. R27 stated,” Yes, R35 keeps asking me for sex, I do not feel safe, I can not defend myself, I told E17 (PRSC) last week, they have not done anything.” ”
  • Failure to apply hand splints or brace to increase range of motion: “R17 was observed on 03/11/12 at 9:30am and 12:00pm sitting in a wheelchair in the doorway. R17’s right hand was observed contractured, flexed position in the palm. R17 was observed with no splint or hand roll on. On 03/12/12 at 10:00am and 2:30pm was observed with no splint or hand roll on. R17 was also observed on 03/13/12 at 11:30am with no splint or hand roll. The minimum data sets date 02/23/12 denote section: G0400 (A). Upper extremity score 1(had functional limited range of motion on one side (shoulder, elbow, wrist and hand). R17 is physically depended on staff member for all activities of daily living. E13 on 03/13/12 at 11:30am stated,” She should have a right hand splint or hand roll on. The care plans are in the chart.”
  • Failure to have medication available: “R30’s blood glucose measured 272/dl on 3/11/12 at 11:30am. E5 (nurse) did not administer [MEDICATION NAME] regular insulin 8 units on the sliding scale. E5 checked the refrigerator for [MEDICATION NAME]medication. E5 on 03/11/12 at 11:30am stated,” There is no [MEDICATION NAME] regular insulin in the refrigerator. She is to get 8 units of [MEDICATION NAME] regular insulin. I didn’t give the [MEDICATION NAME] regular insulin. The pharmacy said the insulin was not ordered.”
