In a summary statement of deficiencies dated 09/18/2015, a state surveyor made a notation during an annual licensure and certification survey concerning the facility's failure to "comprehensively assess the possible cause and implement individualized interventions to manage and control a resident’s pain.” The deficient practice by the nursing staff at Alden Poplar Creek Rehabilitation and Healthcare Center affected one resident at the facility.
The failure was first noted at 10:10 AM on 09/16/2015 upon observation of a resident who “was lying in bed, alert and oriented.” During an interview, the resident stated that they “had not been up out of bed since last week because it is so uncomfortable to be up in the wheelchair.” The resident indicated that “she is having a lot of pain and that if she [the resident] could get it under control, she could be transferred (using the total body lift) into a wheelchair.” The resident indicated that “sitting in her high-back wheelchair is painful.”
The state investigator interviewed the facility’s Assistant Director of Nursing at 3:20 PM on 09/17/2015, who stated that they “are aware of [the resident’s] complaints of pain when up in the wheelchair [...and] that the chair is uncomfortable for [the resident] and she does not like to get up because of it.” The Assistant Director of Nursing stated that the resident’s pain medication “was increased from every eight hours every six hours just today.”
However, the investigator noted that the resident’s 09/17/2015 Comprehensive Pain Assessment “is incomplete it does not assess factors that could be caused or trigger [the resident] to have pain [nor does it identify the resident’s] acceptable level of pain, location, frequency, intensity, aggravating or alleviating factors regarding [their] pain.”
- Failure to Ensure That Residents Receive Proper Care to Prevent the Development of a New Bedsore or Allow Existing Bedsores to Heal
In a summary statement of deficiencies dated 09/18/2015, the state investigator noted the facility’s failure “to update and revise the treatment plan and revise interventions for healing a pressure sore for [a resident at the facility].” The investigator conducted an interview with the facility’s Wound Nurse on 09/17/2050 who stated that “she was only recently started as the Wound Nurse two weeks ago and does not have any formal wound training.”
The investigator noted that the resident’s “right hip dressing had a moderate amount of purulence and serous drainage on it [with] brownish gray with blood mixed drainage.” The investigator also noted a hipbone measuring 2.0 centimeters deep by 0.2 centimeters by 1.0 centimeters. It was also noted that the facility’s TAR (Treatment Administration Record) does not have any orders on how to treat the resident’s wound.
