    In a summary statement of deficiencies dated 10/09/2015, the state investigator noted the facility’s failure “to answer call lights in a timely manner to accommodate a resident’s needs.” The investigator noted Alden Estates of Shorewood directly affected four residents at the facility “reviewed for call light response time.”

    The failure was first noted at 10:30 AM 10/07/2015 when a resident “describe the wait for a call light response at about 20 to 30 minutes [… stating that] ‘If a patient is having [medical emergency], it would be too long the patient would be dead by the time they [the nursing staff] comment. I am claustrophobic! The first time I got here, I requested to be taken to the bathroom, they left me there alone and closed the bathroom door. I pulled my call light, no one came, so I started screaming. My room is all the way to the end of the hallway, they probably did not hear me screaming, but I have my light too, I waited for 30 minutes. The motto here is I will be back but they do not [return]. I felt helpless and it is a bad feeling!’”

    The investigator interviewed two other residents at 11:00 AM on the same day who stated that “the staff is slow in answering call lights.” One resident claimed that their roommate “was taken to the bathroom at night and was left there for an hour [… stating] ‘I cannot help her [my roommate] because I got a broken arm and she cannot walk by herself because of her surgery in her leg (a melanoma reconstruction (surgical) on the right lower extremity).” The roommate added that “it was upsetting. I waited and waited and it is hard to sit on a toilet for an hour.”

  • Failure to Provide Every Resident Environment Free of Accident Hazards and Risks and Provide Adequate Supervision to Prevent Avoidable Accidents

    In a summary statement of deficiencies dated 10/09/2015, the state investigator noted the facility’s failure “to provide supervision to [a resident] identified as being at risk for falls by failing to monitor the resident during toileting.”

    The investigator noted that the failure involved a resident at the facility who previously have a stroke. A nurse assisted a Certified Nursing Assistant in transferring the resident to the bathroom to be placed on the toilet and then left alone with the door closed and the “call light cord draped with a trash bag attached to the end of the cord across the resident’s lap prior to leaving the washroom.

    Even though the resident’s wife remained outside the closed door, the nurse and CNA did not remain in the room. In less than 10 minutes, the nurses for the resident’s “spouse calling for help, and went to find [the resident] on the bathroom floor.”
