In a summary statement of deficiencies dated 10/09/2014, a complaint investigation was opened concerning the facility’s failure “to provide supervision to prevent falls.” This is in response to three separate incidences occurring in October 2014. In one incident, a resident stated that, “I fell in the bathroom I was left alone. I turned to wipe and fell off the toilet. I hit the left side of my face, left shoulder and left hip.”
